Hex was originally to have an extra pre-show before the cinema, with anamatronic chandeliers bats etc... but it was dropped shortly before construction due to cost and time restraints.

Staff training and technical upgrades were mentioned and Merlin's reaction to close rides and talk about procedural changes and training suggest human error was almost certainly the contributing factor. That sounds like Merlin's Senior Management pointing the finger at AT's Ride and Technical Staff...
It was a classic Burley interview, ranting about a subject that she knows nothing about and not letting the actual facts get in the way of her rant. She is a first class idiot.
